Responsibilities
- Manage a primary caseload of hospice patients, providing skilled nursing care in accordance with physician orders
- Perform patient assessments, symptom and pain management, and medication administration
- Document care and maintain accurate records in compliance with Medicare Conditions of Participation, Ohio hospice licensure, and agency policies
- Coordinate care with the interdisciplinary hospice team, families, and community resources
- Provide education and emotional support to patients, families and caregivers regarding end-of-life care
- Participate in care planning, case reviews, and quality improvement initiatives
- Offer PRN coverage during scheduled hours (8a-5p) and respond to changing patient needs
Education
- Graduate of an approved nursing program (Associate, Diploma or BSN)
- Bachelor of Science in Nursing (BSN) preferred
- Active Ohio Registered Nurse (RN) license
