Ambulatory Oncology Nurse Navigator

Department Overview

The Oncology Nurse Navigator will work collaboratively with the Community Hem/Onc teams when a patient is referred to the department and throughout the cancer continuum. An oncology nurse navigator (ONN) is a professional RN with oncology-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an ONN provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The ONN will work with the ONS core competencies as the guide. The ONN competencies to be performed: (a) coordination of care, (b) communication to patient, families, and members of the healthcare team, (c) patient and family education (d) professional role.

 

The Oncology Nurse Navigator will work collaboratively with the Community Hem/Onc teams when a patient is referred to the department and throughout the cancer continuum.

Function/Duties of Position

  • The RN Navigator works with the Providers to ensure that all baseline clinical information, both diagnostic and therapeutic, is gathered together in a systematic fashion prior to the patients visit.  S/he works to ensure that new patients are prepared for the testing, evaluation and potential outcome of the visits ahead of time.
  • The RN Navigator collaborates with the Team Coordinators and Managed Care Coordinators to ensure that all appropriate business related information is gathered prior to the visit, authorizations have been obtained and pre-registration has taken place.
  • The RN Navigator may be called upon to assign tasks to team members after assessing patient needs, staff skills and resources.
  • The RN Navigator collaborates with other members of the team to ensure that patient flow is expedited during clinical practice sessions.
  • The RN Navigator ensures that Provider orders that result from the new patient visit are recorded and executed.  S/he also works to ensure that laboratory and other diagnostic findings are available to clinicians in a timely fashion after tests are performed.  S/he tracks the post visit flow information to ensure that plans of care are executed in a timely fashion.
  • The RN Navigator ensures that when hospitalization is required; those patients and their family members understand prior to admission, what they can expect during the pre-hospitalization, hospital stay and post discharge period.  The RN Navigator is expected to troubleshoot any problems that patients and their families may encounter in preparing for or during their hospital stay.

Essential Nursing Practice Functions for OHSU Registered Nurses

The following provides a summary of the nursing practice essential functions for all RNs at OHSU:

·Develop, coordinate and supervise implementation of the nursing process.

· Demonstrates performance consistent with Departmental, Divisional and Organization expectations and core competencies.

· Assess potential or actual patient health problems.

· Prioritize patient care responsibilities and seek consultation, when indicated.

· Perform area specific skills and follow established protocols.

· Execute Provider orders, seeking clarification, when indicated.

· Recognize and respond to emergency situations.

· Document according to established policies and procedures.

· Coordinate care and/or services with other team members.

· Communicate in a respectful, caring and non-judgmental manner.

· Provide health teaching and counseling which is supportive and/or restorative to health.

·Demonstrates fiscal accountability and the effective use of resources.

·Assume responsibility for professional job-related development, knowledge of advances in technology, use of new equipment and appropriate application of hospital systems.

·Incorporates the principles of OHSU’s Mission Statement into performances.

·Participate in teaching and research missions of OHSU Healthcare.

·Support patient and family centered care across the continuum through service coordination.

·Perform staff member requirements as outlined in the Compliance Section of the Core Competencies.

·Adhere to the OHSU Code of Conduct.

·Identify and prioritize pain and comfort care and adhere to the OHSU policy for pain management. 

Required Qualifications

  • Two years of Adult Hematology Oncology nursing experience within the past five years
  • The RN must have one of the following:
  • Current OCN (Oncology Certified Nurse) certification or
  • Complete 36 cancer related continuing education nursing contact hours each accreditation cycle, per Commission on Cancer
  • BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
  • ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
  • Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
  • Current, unencumbered Oregon State Registered Nurse License 
  • Washington RN license within 6 months of hire
  • BLS at time of hire and must be issued by the American Heart Association (AHA)
  • Must be able to perform the essential functions of the position with or without accommodation

Additional Details

Length of Orientation – Experienced Nurse

    • External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week.
    • Either Transition to Practice (TTP) Program Specialty Fellowship/Fellowship Learning Pathway or Orientation experience for 3 days-26 weeks depending on the care area. An employment service agreement may apply.
