As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. Location: Lincoln Park; hybrid role
Schedule: Monday-Friday with flexible scheduling, 32 patient contact hours minimum (+ 8hrs of administrative time). Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-Call

On-Call: Phone call only, in a rotation every 7-8 weeks for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8.1.24). 24/7/365 physician back up.
