Clinical Pharmacist, MTM- Mississippi

This is a high-impact, patient-centered role supporting medication therapy management within a collaborative and innovative care environment. As a Clinical Pharmacist, you will conduct comprehensive medication reviews, identify optimization opportunities, and recommend evidence-based interventions to improve outcomes for patients with chronic and complex conditions. Working closely with physicians, nurses, and care teams, you will ensure medication use is safe, effective, and aligned with best practices. This position offers remote or hybrid flexibility, depending on location.

Accountabilities:

• Conduct comprehensive medication reviews (CMRs) using clinical data and patient information to develop drug therapy action plans.
• Communicate medication-related recommendations clearly to medical providers, patients, and internal care teams.
• Serve as a clinical pharmacy resource for field-based healthcare professionals, supporting safe and effective prescribing practices.
• Participate in interdisciplinary care team meetings and contribute to ongoing medication optimization strategies.
• Review patient medical records and reports to support clinical outreach and quality assurance.
• Deliver training sessions to standardize and enhance medication therapy management (MTM) practices.
• Stay up to date with evidence-based prescribing, particularly for chronic kidney disease and related conditions.
• Monitor medication trends and help implement personalized medication management initiatives.

• Bachelor’s degree in Pharmacy or PharmD.
• Active pharmacist license and legal residency in the state of practice.
• Minimum 3 years of experience in a clinical, ambulatory care, or managed care setting.
• Strongly preferred: PGY1 residency or BPS certification in pharmacotherapy, geriatrics, ambulatory care, or a related field.
• MTM-specific training or board certification is a plus (e.g., NBMTM, APhA, Powerpak).
• Experience collaborating with interdisciplinary clinical teams and managing chronic care medication needs.
• Excellent written and verbal communication skills, with attention to clinical detail and documentation.
