Bilingual Transition of Care Nurse

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group delivering 24/7 virtual-first and in-home care for children with complex medical needs. We are seeking a compassionate, organized Bilingual Transition of Care (TOC) Nurse to perform post-discharge outreach, coordinate care with families and community providers, and collaborate with an interdisciplinary team to ensure safe transitions from hospital to home.

Skills

  • Licensed RN with eligibility for compact or state licenses
  • Experience working with pediatric populations
  • Care coordination and discharge planning experience
  • Strong telephonic communication and documentation skills
  • Ability to use EMR tools and reporting dashboards

Responsibilities

  • Perform telephonic outreach to caregivers and patients after hospital discharge to complete Transition of Care visits
  • Review and clarify discharge plans with caregivers and assess barriers to successful post-discharge recovery
  • Coordinate care and communicate with PCPs, specialists, hospital discharge planners, and community partners to ensure continuity
  • Leverage dashboards and reporting to prioritize outreach and follow-up activities
  • Collaborate with MDs, APPs, social workers, clinical pharmacy, and community health navigators for comprehensive care
  • Document encounters and care plans accurately in the electronic medical record
  • Escalate clinical concerns and coordinate necessary referrals or interventions

Education

  • Bachelor's degree in Nursing (BSN) from an accredited university required
  • Compact RN licensure or eligibility for state licensure
