Join Grayce as a Virtual Care Provider (NP/PA/1099/Remote) – Redefining Care for Medicare Members
At Grayce, we believe navigating healthcare shouldn’t be so hard. That’s why we’re building a new kind of virtual care experience—focused on bringing clarity, compassion, and continuity to Medicare members and their circles of care.
We’re on a mission to simplify the complex, ensuring individuals and families can access the right support at the right time. Our work began with caregiving support for employees and their loved ones—now we’re extending that care to individuals on Medicare and the people who support them. We’re blending clinical expertise with whole-person care to help people manage health and social needs with less stress and greater confidence.
This is care, reimagined. And we’re looking for dedicated Nurse Practitioners or Physicians Assistants to help us bring it to life.
As a Virtual Care Provider, you’ll play a critical role in our care model by conducting virtual intakes, laying the clinical foundation for ongoing care navigation. You’ll ensure Medicare recipients receive timely, high-quality medical assessments that empower their care journeys, while working remotely with the flexibility and autonomy of an independent contractor.
How You’ll Make an Impact:
Conduct comprehensive telemedicine-based E&M visits to establish care, evaluate medical conditions, barriers to care, and initiate care plans for Medicare recipients. Review and approve care plans developed by Care Navigators—ensuring clinical alignment and tailoring to each member’s unique needs
Partner closely with Grayce’s Care Navigators and interdisciplinary team, supporting whole-person care that integrates medical, emotional, and social needs
Document assessments and clinical findings clearly and efficiently in accordance with Medicare billing standards and telemedicine best practices
Provide clinical leadership and guidance to ensure care continuity and support care navigation for complex cases
Stay current on telemedicine best practices and evolving Medicare care models, contributing insights that help shape Grayce’s innovative care delivery
What You Bring:
Active NP or PA license in good standing, with multi-state licensure required (30–50 states).
Board certification in Family Medicine, Adult-Gerontology, Internal Medicine, or Geriatrics (NP/PA equivalent
Experience delivering patient-centered care to Medicare beneficiaries and/or older adults
Strong knowledge of Medicare guidelines, chronic condition management, and health-related social needs
Active enrollment as a Medicare provider (preferred)
Demonstrated experience using telehealth platforms and advising on or contributing to digital health product development
A collaborative spirit and passion for delivering whole-person care in a virtual environment
Enthusiasm for working alongside care navigators and addressing the social drivers that influence medical outcomes
