Utilizes clinical judgment and expertise to coordinate care management activities for adult pulmonary patients. Performs patient assessments, develops and implements treatment plans, provides education to both patients and their families, and completes discharge bundles prior to discharge and ensures all components of transitions of care are complete. Works closely with the pulmonary team through rounding and daily communication. Serves as a liaison between pulmonary medicine and the inpatient respiratory therapist. Facilitates seamless care for adult pulmonary patients, both outpatient and inpatient, and their families, and ensures effective and timely use of resources to address identified needs. Promotes quality care delivery across the continuum of care.Job Duties:
