Overnight Digital Triage Registered Nurse
Skills
- Active RN license and current clinical license in applicable states
- 2+ years of clinical triage experience, preferably in ER, primary care, or high-acuity settings
- Strong knowledge of chronic disease management and complex medical conditions
- High proficiency in telephone-based clinical assessment and remote care decision-making
- Excellent verbal and written communication and documentation skills
- Comfortable using EMR/clinical documentation systems and telehealth technologies
- Ability to communicate urgent needs across teams and with diverse patient populations
Responsibilities
- Triage and provide clinical support for incoming patient communications via phone and digital channels
- Conduct thorough telephone assessments of symptoms to determine appropriate care pathways and urgency
- Document assessments, care plans, and follow-up instructions in the electronic medical record
- Escalate urgent or high-acuity cases to clinicians or emergency services as appropriate
- Collaborate with multidisciplinary clinical teams to coordinate patient care and ensure continuity
- Assist in developing and improving processes and clinical protocols for virtual triage and urgent care
- Provide culturally appropriate patient and family education tailored to medical literacy level
- Participate in quality improvement initiatives, team huddles, and case reviews
Education
- Associate or Bachelor of Science in Nursing (ADN or BSN) required
- Active RN license in the U.S.; additional state licenses a plus
- Current basic life support (BLS) certification preferred
