- Active NP or PA license and board certification in Virginia
- Strong primary care and geriatric clinical assessment skills
- Ability to develop diagnoses and treatment plans independently
- Comfort prescribing medications and ordering diagnostic tests
- Excellent verbal and written communication with patients and clinical teams
- Experience with in-home care or hospice preferred
- Proficient problem solving and time management for autonomous practice
Responsibilities
- Deliver high-quality, patient-centered primary and palliative care during in-home visits across Fairfax, Prince William, and Loudoun counties.
- Perform physical exams, obtain and update medical histories, and develop individualized care plans.
- Diagnose conditions, develop treatment plans, and prescribe medications and appropriate interventions.
- Order and interpret diagnostic testing as medically necessary.
- Coordinate care and communicate effectively with office staff and clinical operations across internal platforms.
- Foster trust and consult with patients, families, and caregivers; attend quarterly medical staff meetings.
- Manage a panel of patients, including chronic disease management and remote patient monitoring as applicable.
Education
- NP or PA certification required
- Master's degree in Nursing or relevant PA education preferred
- Active state licensure (Virginia)
- Board certification appropriate to role (NP or PA)
