About the Role
As a Health Advocate at Careway, you will be the frontline support for Medicare patients navigating a complex healthcare system. Your mission is to remove barriers—whether scheduling challenges, interpreting doctor instructions, insurance paperwork, or social determinants like transportation needs. Working closely with our physicians, you will build trusting, long-term relationships, providing consistent and personalized support to ensure patients achieve their healthcare goals.
This is a 1099 role. You can choose to work part time or full time. The role is remote.
What You’ll Do
Guide patients through healthcare challenges, clarifying complex medical information in simple terms, coordinating seamless care, and assisting with insurance paperwork.
Create and manage individualized care plans in partnership with Careway physicians, addressing social determinants of health such as transportation, food, home support, and digital access.
Proactively identify patient needs, prioritize actions, and coordinate appointments, referrals, and follow-ups.
Empower patients by improving their health literacy and self-management skills through education and coaching.
Collaborate with our clinical and operational teams, sharing insights to continuously improve Careway’s advocacy-driven care model.
What You’ll Bring
Experience in patient advocacy, care coordination, social work, healthcare navigation, or a related field.
Deep understanding of Social Determinants of Health (SDOH) and experience working with diverse, often vulnerable patient populations.
Exceptional empathy, active listening, and interpersonal skills, enabling you to build trust quickly and maintain meaningful relationships.
Strong organizational abilities and clear communication, allowing you to manage multiple tasks effectively.
Tech-savvy and capable of quickly learning modern EHR systems, digital collaboration tools, and patient engagement apps.
Action-oriented mindset with a commitment to improving patient outcomes and system efficiency.

Nice-to-Haves
Certification in nursing, patient advocacy, social work, or related field.
Background in community health, health coaching, or experience with Medicare populations.
