The listed minimum pay reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

Position Overview

SUMMARY (BASIC PURPOSE OF THE JOB)

Coordinates and oversees the daily operations of the Diabetes Self-Management Education and Support (DSMES) program in accordance with ADA and NJ Nurse Practice Standards. Communicates effectively and works collaboratively with other team members. Uses effective communication to promote learning and behavior change in the diabetes population. Plans, develops, and implements new programs to meet the needs of the patient population and to enhance diabetes self-management education. Works in collaboration with the diabetes care team to maintain and enhance the ADA certification of the DSMES program and ensure that the National standards (NSDSMES) are met and maintained at all times.

MINIMUM REQUIREMENTS

Education:BSN. Certified Diabetes Educator (CDE) certification.

Experience:Three years of experience as an RN in an acute care healthcare facility.

Other Credentials:AHA BLS - Healthcare Provider,Registered Nurse - NJ

Knowledge and Skills:Excellent organizational and verbal and written communication skills. Strong interpersonal skills. Strong working knowledge base of endocrine/diabetic care.

Special Training:Working knowledge of Word, Excel, and Power Point.

REPORTING RELATIONSHIPS

Does this position formally supervise employees? No

If set to YES, then this position has the authority (delegated) to hire, terminate, discipline, promote or effectively recommend such to manager.

ESSENTIAL FUNCTIONS

  • Oversees the planning, implementation and evaluation of the Diabetes Self-Management Education and Support Program (DSMES) at all sites.
  • Arranges and coordinates the activities of the Diabetes Education Advisory Group.
  • Liaises between the DSMES team members, the Advisory Group, other departments, and administration.
  • Monitors and facilitates maintenance of DSMES team member qualifications (CE credits, training, competency, licensure, and registrations).
  • Maintains ADA recognition and participates in the evaluation of the DSMES program effectiveness.
  • Provides educational opportunities at times and places as needed. Participates in developing, updating, and maintaining educational material. Enhances diabetes education by involving patients and families and other caretakers as appropriate in the teaching and learning process.
  • Participates in activities that promote and expand the reputation and services of the diabetes program.
