- Active RN license in Illinois
- Ability to assess and manage end-of-life symptoms
- Strong clinical documentation and charting skills (EMR experience preferred)
- Excellent verbal and written communication with families and care teams
- Valid driver’s license, clean driving record, and reliable transportation
- Basic Life Support (BLS) certification
- Comfort working independently during on-call shifts and making clinical decisions
Responsibilities
- Conduct clinical assessments and develop individualized hospice care plans
- Manage symptoms and pain control, adjusting interventions as needed
- Provide emotional support, counseling, and education to patients and families
- Coordinate care with interdisciplinary team members and external providers
- Complete timely documentation in the medical record and ensure regulatory compliance
- Respond to on-call requests for nights and weekends and perform home visits
- Communicate changes in patient status and escalate concerns appropriately
Education
- Graduate of an accredited nursing program (ADN or BSN)
- Active RN licensure in Illinois
- BLS certification required; hospice/palliative training preferred
