Skills
- Active RN license in applicable state
- Minimum five years of nursing practice experience
- Strong telephonic communication and care coordination skills
- Proficient with EHRs and basic clinical documentation
- Reliable high-speed home internet and solid computer literacy
- Excellent time management, critical thinking, and problem-solving abilities
- Ability to work independently in a remote setting
Responsibilities
- Manage and monitor personal patient census across multiple long-term care facilities
- Coordinate and implement Chronic Care Management (CCM) services for chronically ill patients
- Develop, update, and track individualized care plans and monthly documentation
- Monitor labs, vitals, medications, and diagnostic testing for assigned patients
- Conduct care coordination via phone, text, and email with patients, families, and care teams
- Provide patient education on condition management, medications, and testing
- Identify and close clinical and non-clinical gaps in patient care
- Support and train providers and CCS team members as needed
Education
- Associate or Bachelor of Science in Nursing (ADN/BSN)
- Active RN licensure in the state of practice
