Responsibilities
- Manage a primary hospice caseload and deliver skilled professional nursing care
- Perform comprehensive assessments and develop individualized care plans
- Administer medications and clinically manage symptoms at end of life
- Coordinate care with physicians, interdisciplinary team members, patients, and families
- Document visits and clinical care in compliance with Medicare Hospice Conditions of Participation and agency policies
- Provide education, emotional support, and caregiver training to patients and families
- Ensure regulatory compliance with Ohio hospice licensure and agency standards
- Participate in interdisciplinary team meetings and quality improvement activities
Education
- Graduate of an approved nursing program required
- Bachelor of Science in Nursing (BSN) preferred
- Valid Ohio RN license required
