Hospice Registered Nurse RN - Case Manager - job 1 of 2

Otterbein SeniorLife is a mission-driven, not-for-profit provider of senior living and hospice services. They are seeking a compassionate Hospice RN Case Manager to manage a primary caseload, deliver skilled nursing care, coordinate care plans, and support patients and families in accordance with hospice regulations and agency policies. This role supports Otterbein’s growing hospice ministry and offers both full-time day-shift and PRN opportunities.

Skills

  • Current Ohio Registered Nurse (RN) licensure
  • Clinical assessment and care planning skills
  • Experience with hospice or palliative care principles
  • Medication management and symptom control
  • Strong verbal and written communication
  • Ability to work independently and manage a caseload
  • Proficiency with electronic medical records (EMR)

Responsibilities

  • Manage a primary hospice caseload and deliver skilled professional nursing care
  • Perform comprehensive assessments and develop individualized care plans
  • Administer medications and clinically manage symptoms at end of life
  • Coordinate care with physicians, interdisciplinary team members, patients, and families
  • Document visits and clinical care in compliance with Medicare Hospice Conditions of Participation and agency policies
  • Provide education, emotional support, and caregiver training to patients and families
  • Ensure regulatory compliance with Ohio hospice licensure and agency standards
  • Participate in interdisciplinary team meetings and quality improvement activities

Education

  • Graduate of an approved nursing program required
  • Bachelor of Science in Nursing (BSN) preferred
  • Valid Ohio RN license required
