Responsibilities
- Conduct in-home assessments and functional evaluations
- Deliver evidence-based, personalized recommendations for home modifications
- Educate clients and caregivers on fall prevention and safe mobility techniques
- Engage clients and families in collaborative care planning
- Communicate findings and support process improvement with Clinical Operations
- Complete accurate, timely documentation in the electronic system
Education
- Degree in Occupational Therapy from an accredited program
- Active, unencumbered Occupational Therapy license
- Minimum 2 years of clinical experience
