Responsibilities
Works collaboratively with the Case Management staff and other health care team members to coordinate and execute discharge planning services for patients through the healthcare continuum. Assists in the execution of a plan of care to facilitate a safe discharge in a timely manner and serves as a liaison on community facilities and resources.MC and Case Management documentation standards as indicated in CM policy and protocols.
Qualifications
Education• One of the following is requiredo Bachelor's Degree in Social Work or related fieldo Completion of an accredited Vocational Nursing School program
Experience• 1 year of experience in Case Management with prior assessment and referral experience required
Licenses and Certifications• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required• LVN - Current State of California Licensed Vocational Nurse License for incumbents without a qualifying Bachelor's Degree required
