Responsibilities
- Perform comprehensive clinical assessments and develop individualized hospice care plans
- Manage symptoms and pain control; implement physician orders and comfort measures
- Provide emotional support, counseling, and patient/family education throughout end-of-life journey
- Coordinate care across interdisciplinary team members and external providers
- Complete timely documentation, maintain compliance with hospice regulations and policies
- Conduct regular home visits and communicate changes in patient status to clinical team
- Participate in care conferences and quality improvement initiatives
Education
- Active Registered Nurse (RN) license for the state of Indiana
- High school diploma or equivalent (minimum)
- Associate or Bachelor of Science in Nursing preferred
