ORTHO/NEURO NURSE NAVIGATOR (West Coast)

Full-Time Monday - Friday (NO wknds)

Position SummaryThis role enhances the patient’s experience by providing seamless navigation from pre-procedure scheduling and throughout the care continuum. The Navigator collaborates with all members of the interdisciplinary team to coordinate and consistently manage patient care by serving as a point of contact for patients and families. Systematically and continually performs the functions of assessing, planning, implementing, and evaluating care according to the nursing process.

Responsibilities

Essential Functions• Coordinates patient care through the continuum. Assists with timely access to care, coordinates/facilitates appointments and services, and encourages adherence to treatment plans, protocols, and follow up care. • Assesses patients’ current and future needs, provides access to resources (internal and external), and makes appropriate referrals as needed. • Provides education and information to patients and families related to the healthcare system, multidisciplinary team member roles, plan of care, and available resources.• Establishes and maintains close collaboration with physicians and multidisciplinary team members to ensure seamless patient care and care coordination.• Tracks determined program metrics and quality indicators. Documents patient interactions and progression, and communicates navigation activities to physicians, the care team, and leadership. • Collaborates with the clinical team to obtain or develop patient and family educational materials and resources. • Demonstrates knowledge, skill, and coordination to provide nursing care and guidance to patients from screening to post-operative care and follow up. • Demonstrates effective communication with patients, families, peers, members of the multidisciplinary healthcare team, and representatives of community organizations and resources. • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.• Maintains compliance with all Orlando Health policies and procedures.

Other Related Functions• Actively participates in multidisciplinary planning conferences. • Attends staff development in-services, department meetings, etc.• Establishes and maintains professional role boundaries with patients, families, and the multidisciplinary care team. • Serves as an advocate for patients and families.

Qualifications

Education/TrainingAn Associate of Science in Nursing (ASN) is required. A Bachelor of Science in Nursing (BSN) is preferred.

Licensure/Certification• Maintains license as an RN in the state of Florida.• Maintains current BLS/healthcare provider certification.

ExperienceMinimum of two (2) years of clinical experience in acute care is required. Ortho/Neuro experience is preferred.
