Key Responsibilities:
- Conduct psychosocial assessments to evaluate residents’ emotional, social, and mental health needs.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Provide counseling and crisis intervention for residents and families.
- Advocate for residents’ rights and ensure access to necessary resources and services.
- Coordinate discharge planning, including transitions to home care, assisted living, or other facilities.
- Facilitate support groups for residents and families (e.g., grief counseling, dementia education).
- Collaborate with interdisciplinary teams (doctors, nurses, therapists) to ensure holistic care.
- Assist with Medicaid, Medicare, and insurance documentation for financial and care planning.
- Address elder abuse, neglect, or exploitation concerns with appropriate interventions.
Requirements:
- Education: Master of Social Work (MSW) degree from a CSWE-accredited program
- Licensure: Must be a licensed MSW
- Other: Strong communication skills, empathy, and ability to work in a multidisciplinary team
Compensation and Benefits:
- Salary: $65,000–$75,000 per year
